For patients receiving physiotherapy following an Injury on Duty (IOD) under the Workmen’s Compensation / COIDA system, a referral from the treating doctor or medical practitioner is an important part of the treatment and claims process. Under the Compensation Fund’s physiotherapy requirements, physiotherapy services for a COIDA claim are provided on referral from the treating medical practitioner, and the relevant referral documentation should accompany the physiotherapy account submitted to the Compensation Fund.
The Leanne Samaai Physiotherapy Doctor / Physician Referral Form provides a simple and structured way for the referring practitioner to communicate the employee’s diagnosis, workplace injury, relevant clinical findings and rehabilitation requirements to our physiotherapy team. This helps us develop an appropriate treatment programme that is directly related to the compensable injury while maintaining continuity between the referring practitioner and physiotherapist.
Supporting Accurate COIDA Documentation
Correct documentation is especially important when treating workplace injuries. Compensation Fund requirements provide for medical reports, referral documentation and other supporting information to be submitted through the Fund’s claims process. Our referral form therefore helps ensure that the clinical and administrative information required to commence and manage physiotherapy treatment is captured from the outset.
Once completed, the form can be provided to Leanne Samaai Physiotherapy together with the patient’s available WCA/COIDA claim information and supporting medical documentation. Our team can then proceed with the appropriate assessment and rehabilitation programme, with the focus on restoring function, supporting recovery and facilitating a safe return to work.
